About Your Plan

 

For information about your plan, click on the link that best describes your benefits program:

 

  • If your plan has a Schedule of Benefits that lists the procedures and co-payments, you have a Dental Managed Care plan.
  • If your plan has a Schedule of Reimbursements that lists the covered procedures and reimbursements, you have a Scheduled Benefit plan.
  • If you have a plan where SafeGuard pays a percentage of your treatment and procedures are categorized into three or four types of services, then you have a Dental Indemnity/PPO plan.

Dental Managed Care

 

Dental managed care plans are created to provide preventive and diagnostic care at low or no cost which should reduce the need of more costly treatment in the future.

 

Members like dental managed care plans for a number of reasons:

  • Lower out-of-pocket costs and no confusing percentages to calculate;
  • They may receive the care they need with no calendar year maximums or deductibles;
  • The plans are easy to understand because each covered procedure is listed along with the amount for which the member is responsible;
  • No paperwork and no claim forms are required; and
  • There are no waiting periods.

Access to Care

 

With dental managed care plans, members receive care through a network of contracted dental care professionals who have been pre-screened and meet high quality standards. The selected SafeGuard General Dentist provides day-to-day dental care and members also have the option of receiving care from a contracted specialist. (Please refer to your Schedule of Benefits for referral information for specialty care.)

 

 


 

Scheduled Benefit

 

These plans were created so that participants can receive treatment from any licensed dental care professional. The Schedule of Reimbursements lists the covered dental procedures with the amount that will be paid by SafeGuard.

 

In order to receive reimbursements:

  • Services cannot go over the "calendar year maximum", which is the amount SafeGuard will pay in claims for you and each of your enrolled dependents for that year;
  • Calendar year deductibles must be met, then SafeGuard will reimburse the amount shown on your Schedule; and
  • A claim form will need to be submitted by either your or your dentist.

Indemnity/PPO

 

Dental Indemnity/PPO plans were created to provide you the option of receiving treatment from any licensed dental care professional or through a contracted dentist who has agreed to reduce standard fees for SafeGuard participants.

 

Dental Indemnity/PPO plans share the following characteristics:

  • A "calendar year maximum" (the amount SafeGuard will pay in claims for you and each of your enrolled dependents for that year);

     

  • A deductible, which must be met before SafeGuard pays claims; and

     

  • Claims payment is determined by the percentage shown on your Summary of Benefits. Treatment procedures are categorized into three or four different types (i.e., Preventive, Basic, Major, and Orthodontia, if covered) and percentages vary by category.

Vision Plans

 

SafeGuard provides Vision HMO and Vision indemnity/PPO plans. Both types cover exams, frames, lenses and contacts at low or no cost to you -- and you are not required to "pre-select" a provider when you enroll.

 

Your Schedule of Benefits lists the services your plan covers.

  • Vision HMO plans provide care through a network of highly qualified vision care professionals, and reimbursement benefits for emergency services are also included.

     

  • Vision indemnity/PPO plans give you the freedom to choose any licensed vision care professional or to save on out-of-pocket costs when you receive services from a SafeGuard contracted eye care professional.

SafeGuard & TLCVision Make LASIK Affordable

 

As a member in a SafeGuard vision plan, you and your immediate family members are eligible for a savings of 15% off the usual and customary charges or a 5% off a TLC promotional fee for LASIK at any TLCVision Advantage Network location.
