How does this plan work?

Your organization has chosen a plan with certain materials and/or services covered through a series of allowances in- or out-of-network. Each time you receive treatment, either you or your eye care professional will file a claim with SafeGuard and will be paid based on your plan design.
A Summary of Benefits is included with your enrollment kit - your Summary and the Certificate of Insurance* that you will receive provides all of the information you will need.
*Once enrolled, you will receive a Certificate of Insurance booklet with your ID card. This booklet will give you all of the information you need to effectively access your benefits.

Do I have to choose a vision care provider when I enroll? |
|
There is no requirement to "pre-select" a vision care provider. With this plan you may receive treatment from any licensed eye care professional... but seeing a SafeGuard contracted eye care provider could save you on the cost of treatment.
You will receive a Participating Provider Directory with your enrollment kit. All contracted SafeGuard eye care providers in your area are listed alphabetically by city. Or you can access the most up-to-date information from our website by clicking on the "Dental & Vision Directories" button above.

How do I file a claim? |

In most instances, your vision care office staff will file the claim for you. Both you and your vision care provider will receive an Explanation of Benefits that details how the claim was paid.
If your eye care professional prefers that you file the claim, complete a standard claim form and submit it to:

What is the difference between an optometrist, an optician and an ophthalmologist? |

A. |
Optometrists are doctors who provide routine eye care; they are able to diagnose and treat eye conditions and eye diseases, and can prescribe glasses and contact lenses.
Ophthalmologists are medical doctors who specialize in eye and vision care and the prevention of eye disease and injury; they can perform surgery and the diagnoses and treatment of diseases and treatment of diseases that affect vision as well as prescribe glasses and contact lenses.
Opticians are technicians who make, verify and deliver lenses, frames and other specially fabricated optical devices; they require a prescription from an Optometrist or Ophthalmologist.

I am considering LASIK surgery as an option. Does SafeGuard offer any LASIK discounts? |

As a member in a SafeGuard vision plan, you and your immediate family members are eligible for a savings of 15% off the usual and customary charges or a 5% off a TLC promotional fee for LASIK at any TLCVision Advantage Network location.
